OPNA Survey Information & Permission Form

Spartan families,

The Oklahoma State Department of Mental Health & Substance Abuse, in conjunction with the Oklahoma Department of Education, mandates schools to administer the Oklahoma Prevention Needs Assessment (OPNA) to 6th, 8th, 10th and 12th grade students biannually.

 
 

BPS will administer the survey to students with permission on March 25th

We are asking parents to provide permission BEFORE the student can participate in the survey. After reviewing the information below, if you would like your student to take the survey, please fill out the parental consent form here by Mar 23, 2026

 

It is important to note:

  • Your child’s participation is completely voluntary and confidential.
  • If your child does not participate, he or she will not be penalized or lose any school benefits to which he or she is otherwise entitled.
  • If participating, your child may discontinue the survey or skip any or all questions on the survey without penalty or loss of any school benefits to which he or she is otherwise entitled.
 

Survey Details:

  • The survey is being conducted by the OSU Center for Family Resilience under a contract with the State of Oklahoma. 
     
  • The approximate time to complete the survey is 45 minutes and will be given during the school day by the school counselor for those students whose parents OPT IN.
     
  • Families can read about the full scope and sequence and preview surveys here.
     
  • The purpose of the survey is to gather information needed to plan prevention and intervention programs in our community and schools. 
     
  • Survey questions are related to mental health; alcohol, tobacco, and other drug use; violent behaviors; other health risk behaviors; and related risk and protective factors. 
     
  • The survey is entirely anonymous and confidential. Students will not include their names or any other identifying information. 
     
  • All results from the survey will be presented only in group summary form. 
     
  • If you have any concerns or questions about your student's rights as a participant in this survey, you may contact the ODMHSAS at OPNA@odmhsas.org.
